I am 64 years old, have severe thinning of hair on top of my head and my hairline is receding fast. Don't know the cause...have started wearing wigs...I don't know if that contributes to the hair loss, but makes me feel better anyway.
I'm developing a bald spot in front, just behind hairline.

Yes, it probably would be a good idea to see a derm!
I remember scalp tenderness with alopecia areata, but I don't know if that happens with MPB. It could be a combination of things.
