Looking for advice - 4 year old with alopecia arreata spot expanding

We recently noticed a bald spot about 30mm diameter on my 4 year old son's head. After reviewing some photos we realized it had been there for a couple of months but because his hair was a bit longer we hadn't noticed it. We were fortunate enough to get in with a dermatologist within a week (typically the wait is 6 months in Canada) and he quickly diagnosed it as AA. He wanted to see how it progressed with a review appointment in 3 months. He said there were cream options but said results were never great and there is the typical risks associated with any steroidal creams so we decided to forgo that treatment for now and see where 3 months took us. 

A month later we've remeasured the spot to 35mm x 45mm, though there is some definite growth in the middle, where on initial discovery it was perfectly smooth. 

We are concerned about the expansion, and without ready access to the dermatologist I am left with asking questions on the internet. Is expansion of the bald spot typical? Is it further hair loss due to AA or is it potentially due to friction with his toque, hockey helmet, or some other object? 

Does anyone have any experience with this type of spot growth, or any advice on what to expect? I realize a lot of cases are different but anything anyone can offer would be great!

Also please let me know if "bald spot" is not an appropriate term, I'm not very current on politically correct terminology as such
