I am looking for some opinions and insite...
A new Derm that I went to see this week at Hershey Medical center here in PA who is a Scalp and skin specialist has suggested I try squaric acid treatment.
Has anyone used this treatment and what is your opinion or outcome? I know everyone reacts differently but just trying to see if this is something I want to try.

sorry I have not read many comments above. Did you do sensitization, or started right away with .8%? this seems like a very strong does to me (apologies if you already mentioned it).

I hadn't mentioned it before. I was sensitized in a small area on my arm with the .2%. Initially, I did not have a reaction for about a week or so, then my skin became red and I started to get these small bumps on the sensitized area. It looked like an allergic reaction. That area has since, become one of the large blue/gray bruised looking areas.

I don't understand why you would get sensitized in the arm. The skin in the arm is much more sensitive, so you may have as well put your arm in fire for that matter. did a dermathologist to that? it's hard to believe.
I would recommend you to start it over but well. Let the arm get back to normal - it may take weeks, but it will. Then apply a 2% sensitization in your scalp and wait for two weeks. Then start treatment once a week with .01%, and wait for 16-20 weeks.
you may wonder who am i to say this. I am no doctor, but I suffer from AA. I started using sadbe in june 13 and now no one could tell i have AA. But my derm was very very careful with the treatment. Start slow and be patient. And she'd never ever let me put it on my face (only eyebrows) or any other skin.

I'm kind of wary of trying the SA again. Yes, my dermatologist did sensitizing. You think the dark marks and discoloration will clear up? It been like this since November/December last year.
no wonder you are wary in this case, this means you've had it for 8-12 weeks! sorry to hear that again. but you should wonder about going back to the same derm again.
It should clear up according to what I heard from my derms (i started in new haven but i moved to singapore, so i have met a variety of them). I haver read some research on that and I have never heard about it lasting long term. I just hope you are not a special case!

Hi Becca. I was wondering if you decided to try the squaric acid. My derm has just prescribed it for me, which is really unusual for doctors here in the U.S., but after reading some of the negative results, I am scared to try it for fear of losing the little hair I have left.
If you google "Squaric Acid Protocol" Alopecia Areata, the top hit is the Penn State Hershey Dermatology Squaric Acid Protocol dated Feb 2011. Skip past the section on warts and you see the one for alopecia areata.
It does describe sensitizing with 2% on the inner arm.
I just printed it out to share with my dermatologist who used a different approach with the squaric acid - apply .02% on scalp daily as tolerated and if no effect after a month increase to .04%, It is not having any effect on me after 2 months but my next f/u is in 2 weeks. I plan to ask about this protocal as my provider seems like the type of MD who researches and follows the best scientifically based medical protocols and is also very open to what I'd prefer from a patient perspective.
My dermatologist explained that sensitizing on the arm vs the head would be the same clinically and since it will go on my head eventually it made more sense to use it on the head. Because I had no effect, she increased my dosage to 1% and three weeks later I started to notice regrowth. (dosage seems to differ for everyone). Still fuzz but it's now about 1/4 inch long.
At the same time as the regrowth I had an 2nd infected tooth removed, began using probiotics, got back on a vitamin regime (multi, fish oil, B12 complex) and began alternating the squaric acid with essential oils (1T jojoba + 5 drops each of rosemary, cedarwood, thyme, and lavender) massaged into my head - wrap in a towel for the evening and keep oils on head under a hat the next day before washing out. The oils feel good and they smell wonderful. They also take a bit of the bite off of the itchiness of the squaric acid but do seem to be a slight irritant. I'd had a lot of joint pain after sports activities or even just walking and also some stomach issues all of which also cleared up as the regrowth started to occur.
Next derm appt is still 6 weeks out. We'll see if she suggests staying at 1% or lessen the dosage.
